Chronic Back Pain?
Back pain is divided into neck pain (a.k.a. cervical), middle back pain (a.k.a. thoracic), lower back pain (a.k.a. lumbar) or coccydynia (tailbone or sacral pain) based on the segment affected. The lumbar area is the most common area affected. An episode of back pain may be acute, sub-acute, or chronic depending on the duration. The pain may be characterized as a dull ache, shooting or piercing pain, or a burning sensation. Discomfort can radiate into the arms and hands as well as the legs or feet, and may include numbness, or weakness in the legs and arms.
The majority of back pain is nonspecific with no identifiable causes. Common underlying mechanisms include degenerative or traumatic changes to the discs and facets joints, which can then cause secondary pain in the muscles, and nerves, and referred pain to the bones, joints and extremities. Back pain is very common, with about nine out of ten adults experiencing it at some point in their life, and five out of ten working adults having it every year. Some estimate over 90% of people will experience back pain at some point in their lifetime. It is the most common cause of chronic pain, and is a major contributor of missed work and disability. For most individuals, back pain is self-limiting. In most cases of herniated disks and stenosis, rest, injections or surgery have similar general pain resolution outcomes on average after one year. In the United States, acute low back pain is the fifth most common reason for physician visits and causes 40% of missed days off work. Additionally, it is the single leading cause of disability worldwide.
Nonspecific Back Pain
In as many as 90 percent of cases, no physiological causes or abnormalities on diagnostic tests can be found. Nonspecific back pain can be due to back strain/sprain. The cause is peripheral injury to muscle or ligaments. The patient may or may not recall the cause. The pain can present acutely but in some cases can persist, leading to chronic pain.
Chronic back pain in people with otherwise normal scans can result from central sensitization, where an initial injury causes a longer-lasting state of heightened sensitivity to pain. This persistent state maintains pain even after the initial injury has healed.
Balance issues tend to be among the top contributors of back pain. When someone is off balance while walking, running, golfing, lifting a heavy object, working in the yard, doing their job, it causes someone to strain their back when in awkward positions. Many people don’t realize balance is the issue until they’ve had a balance test. Most medical doctors don’t perform balance tests as part of everyday exams. A “balance” issue isn’t where you wobble around all dizzy or have problems walking. The balance issues we’re referring to is how your weight is distributed across your feet while you do certain things such as standing, lifting, squatting, running, playing sports, etc. As an example, if you’re lifting two handfuls of those plastic grocery bags out of your car, and you’re leaning over to grab them, and improper balance causes your feet to distribute most of your weight to the outside of your right foot, this could cause you to compress your knee joint, causing pain and inflamation for a few days.
